This program serves severely emotionally disturbed youngsters and families. The goal is maintain the client in the most natural environment: home, school and community. Young people with complex mental health issues get the help they need through the NYS Office of Mental Health sponsored Children's Blended Case Management Program which treats children and adolescents who have been diagnosed with a mental health disability and are in need of ongoing support to avert hospitalization or out-of-home placement. Case managers work with the entire family, assessing the specific needs of the child and assisting parents with treatment options, educational placements, housing, and the securing of entitlements. Outreach is a major characteristic of this program as staff go into the homes of their clients in order to fully assess the environment and to tap strengths and resources within the community. Most staff members are bilingual and bicultural, and have received extensive mental health training.
