"The Convent Avenue Family Living Center is transitional housing facility for homeless families sponsored by the New York City Department of Housing Preservation and Development (HPD) and the New York City Department of Homeless Services (DHS). In December 1989, the management of the Convent Avenue Family Living Center was transferred from St. Nicholas Park Civic Association to West Harlem Group Assistance, Inc.

The Convent Avenue Family Living Center is a transitional facility where each family occupies a self-contained apartment with the expectation that permanent housing would be sought and leased within a short period of occupancy. In preparation for permanent living, residents are required to participate in the on-site Housing Readiness Program for independent living. |

The admission process to this Center requires a homeless person to go to the New York City Emergency Assistance Unit (EAU) and/or the Red Cross Association where the family may be later referred to a transitional housing facility such as Convent.

| Convent Avenue Family Living Center’s on-site and referral services include: |

- HPD/Relocation - Provides assistance with permanent housing and relocation services for HPD residents.
- Board of Education - Coordinates school related matters, such as, school registration, placement for regular, special education and high school and attendance.
- Ryan Health Center - The Ryan Community Health Center team consists of a Physician Assistant, Licensed Practical Nurse, Social Worker/Outreach Worker who provide primary health care, mental health counseling, substance abuse prevention services and health maintenance workshops.
- Housing Readiness - The course of study includes budgeting, household management, parenting, money management, conflict resolution, health and nutrition, human relations, housing and community resources.
- Cornell University - Provides classes on nutrition.
- Family Advocacy Integrated Resources (FAIR) - Provides Early Intervention Service referrals for children age 0 to 30 months with developmental delays or disabilities.
- Recreation - Convent Avenue Family Living Center provides ongoing activities for children, such as, arts and crafts, movie time, and games.
- Boy Scout - Have Boy Scout troop on-site funded by Boy Scouts of America
New York Cares Volunteers - Assists with transportation and tickets for special events, serving 25-45 children per trip.
- Hospital Audiences, Inc. - Provides housing readiness aids and independent living skills workshops; also, tickets to special events.
